Concerns will only be addressed if the form is fully completed. No phone calls, texts, emails, or verbal conversations will be accepted or reviewed without properly submitting the concern form below.

Once the form is submitted, the MCMS Board will review the concern and email a response to the applicant.

All rulings are considered final. To appeal the first ruling from the MCMS Board, a 2nd concern form must be submitted from an additional person who is not the original applicant.
